Written by Chee Wai Siong Wesley, Director & Chief Sports Physiotherapist at Physio & Sole Clinic

What is Shockwave Treatment?

Shockwave treatment, also known as Extracorporeal Shockwave Therapy (ESWT), is a safe, non-surgical physiotherapy treatment that uses high-energy acoustic waves to:

  • Stimulate the body’s natural healing process
  • Improve blood circulation and tissue regeneration
  • Provide effective relief from chronic or acute pain

Each session lasts about 10–20 minutes, with most patients experiencing results after 4–8 treatments.

Conditions Treated with Shockwave Therapy

At Physio & Sole Clinic, we use Radial Shockwave Therapy (RSWT) to treat a wide range of musculoskeletal conditions, including:

  • Plantar fasciitis & heel pain
  • Tennis or golfer’s elbow
  • Achilles tendinopathy
  • Patellar tendinopathy (jumper’s knee)
  • Calcific shoulder tendinopathy
  • Frozen shoulder
  • Heel spurs & Morton’s neuroma
  • Chronic tendonitis & trigger points

Whether you’re an office worker, athlete, or simply dealing with nagging pain, shockwave therapy may be the key to long-lasting relief.

How Does Shockwave Treatment Work?

Shockwaves are delivered directly to the affected area using a handheld applicator:

  • Stimulates Healing – Promotes blood flow and metabolism in damaged tissues.
  • Reduces Pain – Lowers pain-sensitising chemicals in the body.
  • Accelerates Recovery – Triggers growth factor release to repair soft tissues.

What is Radial Shockwave Treatment (RSWT)?

Radial shockwave treatment is the most commonly used form of shockwave therapy in physiotherapy clinics, and the only type we perform at Physio & Sole Clinic.

How it works: A small projectile is fired inside a handheld device, creating shockwaves that spread outwards in a “radial” pattern.

Best for: Surface-level or broader treatment areas such as plantar fasciitis, tennis elbow, or general tendon pain.

Key benefit: Effective for treating widespread or less deep conditions, with minimal discomfort and no downtime.

What to Expect During a Shockwave Treatment

Before Your Session

Your physiotherapist will begin by identifying and marking the exact area of pain or injury. A conductive gel is applied to the skin, which helps transmit the shockwaves effectively into the tissues. There’s no need for anaesthesia or special preparation, so patients can come in and walk out on the same day.

During the Session

  • Using a handheld applicator, your physiotherapist will deliver a series of short, high-energy shockwave pulses directly to the affected area. Most patients describe the sensation as a tapping or vibrating feeling. While some mild discomfort may occur (especially over sensitive spots), the intensity is always adjusted to your tolerance level.
  • Each session typically lasts 10–20 minutes, depending on the condition and treatment area.
  • Most patients require 4–8 sessions for optimal results, scheduled about once a week.

Immediately After

It’s normal to experience mild soreness, warmth, or redness in the treated area. This is part of the body’s healing response and usually subsides within a few hours. Unlike surgery or injections, there is no downtime, and you can return to normal daily activities immediately.

Recovery Guidelines

  • Avoid strenuous activity for at least 48 hours after treatment.
  • Hold off on high-impact exercises (e.g., running, jumping) until two weeks after completing your course of shockwave therapy.
  • Your physiotherapist may recommend complementary treatments such as stretching, strengthening exercises, or orthotics to support long-term recovery.
