Metatarsalgia Forefoot Pain
前足疼痛

Podiatrist performing manual therapy to address forefoot pain, promoting relief and improved foot health.

What Is Metatarsalgia (Forefoot Pain)?

Metatarsalgia, also known as forefoot pain, refers to discomfort or inflammation in the ball of the foot, located between the arches and toes. This condition often results from excessive pressure on the metatarsal bones and is commonly associated with high-impact activities, ill-fitting footwear, or underlying health conditions. It can cause pain, tenderness, and difficulty in daily activities such as walking or running.

Causes of Forefoot Pain

Lifestyle Factors:

  • Prolonged standing or walking on hard surfaces.
  • High-impact activities like running, jumping, or sports.
  • Wearing ill-fitting footwear or high heels.

Health Conditions:

  • Arthritis (rheumatoid or osteoarthritis).
  • Gout and Morton’s neuroma.
  • Sesamoiditis (inflammation under the big toe joint).
  • Diabetes or other chronic conditions.

Biomechanical Issues:

  • Foot deformities like bunions, claw toes, or hammertoes.
  • Abnormal gait mechanics such as overpronation or in-toeing.
  • Reduced forefoot fatty padding due to aging.

Symptoms of Forefoot Pain

  • Sharp, burning, or aching pain in the ball of the foot.
  • Numbness or tingling in the toes.
  • Swelling or tenderness in the affected area.
  • Pain worsens with activity or walking on hard surfaces.
  • Feeling like there’s a pebble in your shoe.
  • Relief from pain when resting or sitting.

How Is It Diagnosed?

Our podiatrists at Physio & Sole Clinic will perform a comprehensive assessment that includes:

  • Biomechanical and musculoskeletal evaluations to identify pressure points.
  • Gait and posture analysis to detect abnormalities.
  • Diagnostic imaging such as X-rays, ultrasounds, or MRIs (if required) to confirm the condition and rule out fractures or other complications.

Treatment Options

Conservative Approaches:

  • Footwear Modifications: Advice on supportive, cushioned shoes with a wide toe box.
  • Orthotics & Insoles: Customised insoles, metatarsal pads, or offloading solutions.
  • Stretching & Strengthening Exercises: To improve foot muscle function and reduce strain.
  • Activity Moderation: Adjusting daily routines to reduce strain on the forefoot.

Advanced Interventions:

Extracorporeal Shockwave Therapy (ESWT) being applied to a patient's heel, a treatment option for Metatarsalgia and forefoot pain.

  • Extracorporeal Shockwave Therapy (ESWT): Stimulates tissue healing and reduces inflammation.
  • Surgery is considered only when conservative treatments fail.

What Happens If Left Untreated?

If untreated, metatarsalgia can lead to:

  • Persistent pain spreads to other parts of the foot, ankle, knee, or hip.
  • Gait changes or limping, leading to further biomechanical issues.
  • Development of irreversible deformities such as claw toes or bunions.
  • Progression to chronic conditions like avascular necrosis or joint stiffness.

Prevention Tips

  • Wear well-fitting, supportive footwear with proper cushioning.
  • Avoid prolonged periods in high heels or barefoot walking on hard surfaces.
  • Use orthotic inserts for extra support.
  • Maintain a healthy weight to reduce pressure on your feet.
  • Stretch and strengthen foot muscles regularly.
  • Exfoliate calluses and keep the feet moisturised to prevent additional pressure points.
